We dedicate an expert team – consisting of a transition specialist and behavioral coach – to each Resilience participant. Over the course of the program, the same specialist and coach meet individually each week with the program participant.

What is a transition specialist?
All AbilTo transition specialists are licensed clinical social workers. They are specially trained to help participants understand and adopt new behaviors suitable for moving ahead with life after a medical event.
What is a behavioral coach?
Behavioral coaches are specially trained to assist participants apply useful new life skills.
What is the advantage of working with a team?
